A cracked.com collection of some very funny mistakes which went out live on the home shopping network. Most involve some form of injury.
I had seen the katana one (which is classic), but the rest are new.
Incidentally, I kind of hate Cracked's trend of nearly every article following the "THE 10 MOST (blank) (blank) OF ALL TIME" format. But they do have some pretty good stuff now and then.

I don't know which is my favorite, the horse is great, but the first guy who falls on the ladder, at the very end the lady says "oh hes moving hes ok". and the ladder ladder salesman made a rather graceful recovery, verbally that is. The rest are just hilarious.
Yeah the one with the guy who almost breaks his neck on the folding ladder was kind of amazing in the way that he didn't miss a beat... just kept on talking even after the whole thing collapsed.
Apparently Mike Rowe (the guy who hosts "Dirty Jobs") used to sell crap on QVC. Here's a clip of him selling a purse. And by selling a purse, I mean talking about being sexually attracted to Disney's "The Little Mermaid":
